Disclaimer and Liability Waiver

I, [Parent/Guardian Name], hereby acknowledge that I have read and understood the terms and conditions of the MCWS After School Tarbiyah program. I understand that the program is designed to provide Islamic education and spiritual growth, and I release and hold harmless MCWS, its officers, directors, employees, and agents from any and all liability for any injuries, damages, or losses that may occur to my child during the program.

Assumption of Risk

I understand that there are risks associated with participating in the program, including but not limited to physical injury, emotional distress, and exposure to infectious diseases. I assume all risks and hazards associated with the program and release MCWS from any liability.

Medical Emergency Policy

In the event of a medical emergency, I authorize MCWS to provide basic first aid and seek medical attention if necessary. I understand that MCWS will make reasonable efforts to contact me in the event of an emergency, but I also grant permission for MCWS to make decisions regarding my child's medical care if I am unavailable.

Payment and Refund Policy

I understand that the registration fee is non-refundable. Monthly fees are due before the 5th of each month, and late payments will incur a $20 processing fee. I understand that MCWS reserves the right to can

Code of Conduct

I understand that my child is expected to follow the code of conduct outlined by MCWS, which includes respecting teachers, staff, and fellow students, and adhering to Islamic values and principles.

Michigan Law Compliance

I understand that MCWS is compliant with Michigan laws regarding after-school programs, including but not limited to:

- Michigan Compiled Laws (MCL) 722.113 - Child Protection Law

- MCL 333.9201 - Health and Safety Standards for School-Age Programs

MCWS After School Tarbiyah Program Bylaws and Policy

I have reviewed  the MCWS After School Tarbiyah program bylaws and policy posted on MCWS website (subject to revisions) which includes but is not limited to:

- Program goals and objectives

- Admission and enrollment policies

- Attendance and punctuality policies

- Discipline and behavior management policies

- Health and safety policies

- Confidentiality and privacy policies
