Acknowledgement

I acknowledge that the activity of volleyball involves risks that include but are not limited to personal injury and damage to personal property. I confirm that I am properly equipped and physically able to participate in this activity. I understand that I should consult a physician prior to undertaking any physical exercise. I further confirm that I have no medical, physical or other condition that might preclude my participation or result in a risk of injury from such participation. I hereby assume all risks arising from or associated with my participation in this activity and I hereby release The MECCA Center, its employees, organizers, members, participants and any associated agents from any risks, injuries, damages, claims and liability whatsoever which may arise from, during, after or in any way related to my participation. This release is applicable even though any of them may have been negligent. I agree to pay the costs of any emergency services for myself that may be necessary. I agree to follow the rules and regulations of the activity as laid forth and previously communicated, and conduct myself with good sportsmanship. I understand that I may be removed at the discretion of program directors for violation of these rules.
