Acknowledgement

I/we realize that participation in ICNE Ramada Camp could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, l/we (the student and parent/guardian) assume all risks related to the use of any and all spaces used by Islamic Center of New England (ICNE). l/we agree to release and hold harmless ICNE instructors, staff members, and facilities used from any cause of action, claims, or demands now and in the future. l/we will not hold ICNE liable for any personal injury or any personal property damage, which may occur on the premises before, during or after program. Furthermore, l/we agree to obey ICNE Ramadan Camp program and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by ICNE. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our program, l/we agree to report the unsafe conduct or conditions to the ICNE staff as soon as possible. Waiver and Release. You hereby release and forever discharge and hold harmless ICNE and its agents from any and all liability, claims and demands whatsoever kind of nature, either in law or in equity, which arise or may hereafter arise from your child’s attendance in ICNE Ramadan Camp program. You understand that this releases discharges ICNE from any liability or claim that you may have against ICNE with respect to any bodily injury, illness, or other issue that may result from your child’s participation in the program, whether caused by negligence of ICNE or the employees or volunteers, or other agencies or by the condition of the facilities or areas where ICNE Ramadan Camp programs are being conducted. Assumption of Risk. You further understand that your child’s participation may expose him/her and others to unavoidable COVID-19 community spread. As such, you hereby expressly and specifically assume the risk of injury or other harm, and also expressly release ICNE from all liability, illness, or other issue resulting from or in any way related to your child’s participation.
